PD1 BEHAVIORAL HEALTH SERVICES

11-Unit apartment complex provides supported housing for people with serious or persistent mental illnesses.  Independent apartments are designed to provide housing to those individuals who suffer from serious and persistent mental illness and who require minimum assistance to maintain an independent lifestyle.

Appalachia Court Apartments

Psychological Rehabilitation provides assessment, medication education, opportunities to learn and use independent living skills and to enhance social and interpersonal skills, family support and education, vocational and educational opportunities, and advocacy to individuals with mental health, substance use, or co-occurring disorders in a supportive community environment focusing on normalization.  It emphasizes strengthening the person’s abilities to deal with everyday life rather than focusing on treating pathological conditions.

Independence House

Community Connections provides a continuum of services to children, youth and adults with intellectual or developmental challenges and disabilities.

Community-based child and adolescent services for Lee, Scott, and Wise Counties, and the city of Norton.  Location for Infant and Toddler Connection of DILENOWISCO, Prevention Services, and VALUES Therapeutic Foster Care.

Four Seasons Greenhouse

Residential Services provide overnight care in a group home with an intensive treatment training program.  The Group Home provides 24 hour supervision for individuals who require training and assistance in basic living functions such as meal preparation, personal hygiene, transportation, recreation, laundry, and budgeting.

Polly Summit

The Center offers Crisis Assessment and Evaluation services for adults (18+). The goal is to safely provide a therapeutic environment for individuals who come into contact with law enforcement and are in the midst of a mental health crisis and/or experiencing substance abuse issues.

CITAC

Crisis Intervention and Referral Services are provided by an experienced clinical team 24 hours per day. Services provided are evaluation and placement for individuals needing inpatient care, community based mobile crisis counseling and crisis walk in program where individuals experiencing a crisis can receive care for up to 23 hours when inpatient care isn’t required. All programs work to successfully transition individuals back to their communities and provide referrals to outpatient levels of care.
