Case Management Services
Case management provides a variety of community-based services designed to respond to the unique needs of individuals with mental health, substance use disorders, or co-occurring disorders. Services include identifying and reaching out to individuals in need of services, assessing needs and planning services, linking the individual to services and supports, assisting the person directly to locate, develop, or obtain needed services and resources, coordinating services with other providers enhancing community integration, making collateral contacts, monitoring service delivery, and advocating for people in response to their changing needs.

Day Support Services
Services are provided in a group center-based setting and in natural settings in the community. The program focuses on individualized activities, supports, training, and supervision to people with intellectual and developmental disabilities. Day Support Services are provided through the Adult Activity Center and offer training in areas such as building natural supports, volunteerism, independent living skills, and basic self-care skills.

Residential Support In-Home Services
Residential Support In-Home Services offers training, assistance, and supervision within the individual’s home. Services are person-centered and may include skill-building related to personal care, daily living skills, mobility, communication, money management, monitoring health care needs, developing the ability to manage challenging behaviors, and community access. Services are offered days and evenings.

Sponsored Residential Home Services
Sponsored Homes provide an opportunity for community family living for individuals with developmental/intellectual disabilities. This service mirrors those offered in the In-Home program that is person-centered while working on training and support objectives yet also provide 24-hr. monitoring for health and safety. The goal for the individual placed in the sponsor home is that individuals would have a “life like yours.”

Park Place is a group home for individuals with developmental and intellectual disabilities that help persons achieve independence within the community. Services are person-centered, which may include skill-building related to personal care, daily living skills, mobility, communication, money management, monitoring health care needs, developing the ability to manage challenging behaviors, and community access.

Pierce House and Polly Summit are group homes for individuals with serious or persistent mental illnesses that help individuals achieve independence within the community. Service plans are personalized so individuals may work toward achieving desired goals and provides support in daily living skills and community access.

Appalachia Court Apartments
Appalachia Court is an 11- unit apartment complex that provides supported housing for individuals with serious or persistent mental illnesses that require minimal assistance to maintain an independent lifestyle. This service provides support for community living and daily living skills.

Employment Services
Services provide support and work opportunities to groups and individuals in non-residential settings.

  • Sheltered Employment Services are designed to assess and develop employee strengths at an integrated worksite. Work is provided in a setting that is for people with disabilities who are not ready, are unable, or choose not to enter into competitive employment in an integrated setting. This service includes the development of social, personal, and work-related skills based on an individualized services plan.
  • Supported Employment provides paid employment to an individual placed in an integrated work setting in the community. The individual receives intensive on-the-job training and support, which may include transportation, job-site training, counseling, advocacy, and any other supports needed to achieve competitive employment. The service is a critical link to business and industries in search of qualified employees.
